Spain consults on Draft Order updating the Health Cohesion Fund regulations

On September 24, 2026, the Ministry of Health submitted a Draft Order updating Annexes I-IV of Royal Decree 1207/2006 on the Health Cohesion Fund (FCS) to the hearing and public information procedure.

The Health Cohesion Fund is intended to support equal access to public healthcare services across Spain. Under Royal Decree 1207/2006, the Fund compensates Autonomous Communities for care provided to patients residing in another Autonomous Community and referred to designated Benchmark Centers, Services, and Units of the National Health System (CSURs). The Royal Decree also provides that the diseases, treatments, technologies, and procedures eligible for compensation must be listed in its annexes, together with the reimbursable amounts in each case.

The update would revise the diseases, treatments, technologies, procedures, healthcare processes, and reimbursable amounts covered by the FCS to reflect current NHS needs and CSUR designation decisions. Changes are expected across endocrine, gastrointestinal, endoscopy, extracorporeal treatments, neurology and neurosurgery, neurovascular, in-vitro diagnostics, and some other areas. The revised amounts would apply to eligible care provided in 2025.

Some examples of reference diseases, treatments, technologies, or procedures to be included are provided below:

  • 89 "Complex intracranial aneurysm and cerebral revascularization in adults"
  • 94 "Pediatric renal tumors”
  • 98 "Endoscopic treatment of complex early gastrointestinal neoplasms"
  • 99 "Surgical treatment of borderline resectable pancreatic cancer in adults"
  • 101 "Neonatal and pediatric ECMO transport"

Annex II.A “Ambulatory procedures” is proposed to be updated by incorporating several new procedures, for example, 111 "Clone-specific PCR for diagnosis".
